Job Description
The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the College of Licensed Practical Nurses of British Columbia, and according to agency operating policies and standards and unit specific protocols.
According to the established policies and procedures of the organization, and in collaboration as a member of an interdisciplinary team; is responsible for the daily nursing care, safety and general well-being of residents, performs assessments, plans, implements and evaluates nursing care within the care home including the special care unit.
Duties and Responsibilities
1. Assesses resident’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs within the care home, including the special care unit.
2. Provides a supportive environment for residents. Promotes resident/family involvement in health care decisions; provides information and opportunity to discuss concerns; respects personal choices.
3. Implements and/or contributes to the development and modification of the individualized plan of care, including attending and providing input into Resident Care Conferences; interdisciplinary care planning meetings and family conferences.
4. Performs nursing care and procedures according to identified policies and procedures, technology and agency standards and evaluates outcomes. Records observations and reports problems and/or changes to designated staff. Administers medications to assigned residents according to policies and procedures.
5. Provides personal care to residents, including assisting with toileting, bathing, dressing/grooming, feeding and oral hygiene.
6. Performs admissions, discharges, and transfers of residents according to agency policies and procedures.
7. Documents and updates all relevant care and procedure information to ensure resident safety and comfort.
8. Operates equipment related to resident care such as bathtubs, scales, and mechanical lifts.
9. Assists in maintaining a safe and clean environment for residents and staff.
10. Assists in the orientation of new staff, students and program evaluation activities, education sessions, team conferences, meetings and committees.
11. Provides emergency care as needed until RN and/or emergency services arrive.
12. Performs other related duties as required.
Qualifications and Experience
Education, Training and Experience
Graduate of a recognized practical nursing program or equivalent combination of education, training and experience. Current full practicing licensure with the College of Licensed Practical Nurses of British Columbia (CLPNBC).
